Sacraments - First Reconciliation and First Communion

If your child has been baptized, is entering second grade or above and attended Faith Formation for 1 year in good standing, please reach out to Mary Queen directly for a link to register for Sacraments.

*If your child is NOT BAPTIZED, or baptized into another faith tradition (Lutheran, Methodist, etc.), and is age seven or older, you will need information about the RCIA program adapted for Children of Catechetical Age, please contact Carol Stuart.

Virtual Learning Statement

“By registering your child(ren) for faith formation you give permission to the parish to provide its catechists with access to your child(ren) for the purpose of catechesis. Depending on the circumstances and the program, this access may be face-to-face in a parish facility, or may be on a virtual platform approved and overseen by the parish, or both.  All catechists have been cleared by the Archdiocesan Office of Child and Youth Protection to work with minors and have completed all required safe environment training.”

Medical Consent/Liability Waiver

In the event of an emergency, I hereby give permission to the staff of Mary Queen Catholic Church to seek emergency medical transport or treatment for my child(ren) named above. I wish to be advised before further care is given by the hospital or doctor and I understand that all financial obligations are my responsibility. If I cannot be reached please contact the person listed above.

In consideration of my child's participation in Faith Formation, I agree on behalf of myself, my child(ren) named above, and our heirs, successors, and assigns, to indemnify, hold harmless and defend the Daniel Cardinal DiNardo, Archdiocese of Galveston-Houston, Mary Queen Catholic church, its pastor or any representative of Faith Formation or Yourth Ministry (collectively the "indemnitees") from any and all injuries, losses or claims arising out of my child's participation in the program. I further agree that if anyone on my child's behalf makes a claim against the indemnitees, I will indemnify, save, and hold harmless each of the indemnitees from any litigation, expenses, attorney fees, loss, liability, damage or cost which may incur as a result of such claim.

Video/Photography Consent

As parent/guardian, I understand that promotional pictures and videos (individual and group) may be taken during Faith Formation Classes and/or Youth Ministry Activities. I give permission for my child(ren)'s (named above) pictures to be used for church promotional materials such as newsletters, web pages, social media, calendars, power point presentations, or videos to promote or highlight these classes or activities.

Faith Formation Handbook Acknowledgement

I am the parent/guardian for the child(ren) listed above. I have read the Elementary Faith Formation Policies and Guidelines (click here) or had them read to me. I fully understand and accept these policies and guidelines knowingly, freely and willingly.

Parent Volunteer Opportunities - 

Many volunteers are needed to make this program a success. Please share your time and talent to work with an age group that you would enjoy. Please check your choice(s) now, so that we may alert you to training sessions.

2025 - 2026 Faith Formation Registration Fees:

Parishioner - $70 per child (Grades K-5th)

Non-Parishioner - $100 per child (Grades K-5th)

NOTES on FEES:

• You must be registered in the parish at least 6 months prior to registering for Faith Formation for parishioner fee.
• The pastoral policies of this parish recognize that each student has the right to Catholic formation and no student will be denied participation due to funding. Please contact your program director for more details.
